ElementOne has surveyed the Chief Executive Officers of more than 1,200 Federally Qualified Health Centers and health center “look-alikes” since 1991 collecting information about compensation practices and governance.  More than 300 health center have participated in the survey, from nearly every state and, Puerto Rico and the Virgin Islands.

As with many industries, compensation increases with size.  Users of the survey report can determine what the model “predicts” for their base salary and total cash compensation based on health center revenue.  The survey also provides information on recent salary adjustments, incentive eligibility and payouts, retirement programs, employment contracts, and compensation governance policies.

This comprehensive survey report examines the practices of the participants, which represent about 10% of all health centers.  Findings for 2019 include:

  • Base pay increased at an annual rate of more than 7.0% over the last two year, with total cash compensation (base plus bonus) increasing at a comparable rate.
  • FQHCs have historically had a higher level of gender pay equity in the CEO position than the national average, but while earnings for female CEOs have not caught up with the gains of men.
  • Incentive programs are not the norm. Fewer than  half of health centers offer them, and they are not particularly generous — the average payout is less than 7% of base pay.
  • Less than 5% of the participants maintain a “best practice” governance program.
  • Nearly 40% of the participants have internal staff conduct compensation research, a serious concern and not a best practice technique.
  • The full board of directors approves CEO compensation at 91% of participants.
